Tramadol is available for you to take orally, or it can be injected into muscle or a vein. It is an atypical nonopioid that works like the naturally produced opiates to slow down the manufacture of neurotransmitters. This slows down or stops the brain from processing information from the central nervous system about pain. Thus Tramadol produces pain relief.
Conventional Tramadol is an immediate-release drug requiring a repeat dose four or six times a day. The extended-release form only requires one tablet a day for relief from light to moderate pain. You need a gradual build-up to achieve a minimum level of Tramadol in the blood stream. Steady state is then maintained over the required period of time. The drug level is then managed down again. Unless this process is managed with care, pain relief can be delayed and the side-effects are more common.
Talking to your doctor about Tramadol
Before you take Tramadol consult your doctor to ensure there will be no adverse interactions between Tramadol and any other drugs, health products and dietary supplements you may be taking. For example, do not mix Tramadol with products to relieve colds, flu symptoms or allergies. More importantly, do not take any other pain killers, sleeping pills or pills to help you deal with anxiety or depression.
Since you are proposing to take Tramadol, you are already in pain. Pain is nothing more than a warning sign, a symptom of an underlying medical problem. If you have not already seen a doctor about the cause of this pain, you should do so. Treating the cause of the pain is always better than treating the symptom.
Before taking Tramadol
Do not take Tramadol unless you have first discussed the risks with your doctor if you have:
- drug or alcohol dependence;
- a history of seizures;
- a recent head injury;
- any metabolic disorder;
- liver or kidney disease requiring you to be on dialysis; or
- a history of mental illness including one or more suicide attempts.
If you are planning to become pregnant or are already pregnant, Tramadol can cause serious or, even, fatal side-effects to your baby while in the womb or as a new born. Do not use Tramadol to control pain during pregnancy or during labour. Further, if you take Tramadol immediately after giving birth, it can pass into breast milk and threaten the health of your baby.
Tramadol should not be given to any person under the age of 18 years.
Dosage
If there is anything you do not understand about Tramadol, always ask your doctor or pharmacist to explain. In particular, tell your doctor if Tramadol at the prescribed dose is not helping your pain. Do not increase the dose on your own initiative.
Tramadol is supplied in 50mg tablets for immediate release, and as 100mg, 200mg and 300mg for extended release. If you are using the immediate release tablets, the standard dosage dose is between 50-100mg every 4-6 hours. The maximum safe dose in any 24-hour period is 400mg.
The standard dosage for the extended release tablets is 100mg daily. The daily maximum is 300mg. Do not crush, chew or break open the extended release tablets or capsules. Swallow them whole with plenty of water.
Should you forget to take a dose, never double the next dose to "catch up".
As to storage, keep the tablets at room temperature in an airtight container, avoid excess heat and moisture, and keep Tramadol out of the reach of children.
Possible side-effects of Tramadol
Most people take the drug without side-effects, or Tramadol side-effects prove mild and transitory. The most common are:
- blurred vision, dizziness and drowsiness;
- headache;
- nausea and vomiting.
If any of these effects continue for a worrying length of time or grow worse, consult your doctor.
If you experience any of the following symptoms, call a doctor immediately:
- in rare cases, seizures and convulsions are experienced by those who have alcohol or other drugs in their system, head injuries, epilepsy, etc.;
- symptoms that suggest an allergic reaction including itching, heavy sweating, a rash or a blistering of the skin, etc.;
- shallow breathing;
- palpitations or racing pulse;
- dry mouth;
- diarrhoea;
- insomnia
- visual disturbances.
